All team members will need to fill out this Medical Release Form and put it in a sealed envelope to give to your Team Manager/Coach to keep in a Ziploc bag in each team's equipment bag throughout the season. This information will remain sealed (for confidentiality reasons) unless it is necessary to use it. If it is necessary to use it, it will be returned to the parent to put in another sealed envelope to go back into the equipment bag.
